Probably a shift fork worn really badly....That or a detent ball out of place on the shift rail.
I found the problem. The threaded collar that holds the shifter in place had loosened up causing enough verticle movement to allow the shifter to raise up, which then allowed the rear shift fork to slide forward and get locked up in gear. It's all back together and working fine now.
